I hereby consent and authorise Bold Eye Beauty to perform a makeup application.

I give permisson to the makeup artist named above, to apply makeup products to my skin. I have made them aware of any allergies/sensitivities I may have. I am fully aware of the risks and hazards involved and these have been explained to me.

Cancellation fee of 50% of the reserved service amount will apply if you do not cancel your appointment at least 24 hours prior to your appointment or fail to cancel at all i.e "No Show".
